What is a Concept 4D orthosis?

The Concept 4D orthosis is a dynamic corrective positioning orthosis used after obstetrical brachial plexus palsy (OBPP).

Aid for children with arm plexus palsy

Concept 4D orthosis (Pohlig/Bahm)

We have developed a dynamic corrective positioning orthosis for children with a condition following obstetrical brachial plexus palsy (OBPP). It consists of individual modules that are used depending on the required mode of action. Due to its special design, the Concept 4D orthosis can act in four dimensions simultaneously:

  1. Shoulder: It supports the shoulder in external rotation with the upper arm in the best possible position (adduction).
  2. Elbow: The elbow joint remains freely movable
  3. Forearm: It corrects the supination or pronation malposition in the forearm statically or dynamically
  4. Hand & thumb: If necessary, it statically positions a drop hand and a collapsed thumb.

Construction of the Concept 4D orthosis

The Concept 4D orthosis consists of the following modules:

  • trunk module for stable adaptation of the orthosis to the body (optional with PohliSlide module).
  • optional PohliSlide module for adjustable, dynamic external rotation correction in the shoulder
  • forearm module with wrist closure system and optional PohliSlide module for dynamic support of forearm supination (outward rotation)
  • elbow joint to accommodate the forearm module and free movement into flexion
  • facilitates tightening with separate tightening technique between forearm/elbow module and trunk module

Advantages of the Concept 4D orthosis

The great advantage of the Concept 4D orthosis is that it can be optimally adapted to the child's individual situation. The settings for the shoulder and elbow joints can be varied dynamically. If necessary, movement limitations can be set. Thanks to the individual modules, the Concept 4D orthosis can be donned and doffed separately, allowing even more targeted adjustment of the corrective effect.

Thanks to the 3D printing process, we can use an extremely lightweight, air-permeable, flexible and at the same time robust material for the orthosis. The Concept 4D orthosis has an appealing design and color for children and is very user-friendly.
